ANONYMOUS WHISTLEBLOWING FORM

This form can be used for fully anonymous whistleblowing at Sanok Rubber Company SA.

Rules of anonymous whistleblowing

• all notifications are sent to the Ethics Officer at Sanok Rubber Company SA, whose activities are described in detail in the Code of Ethical Conduct,

• if a report concerns a Member of the Management Board of Sanok Rubber Company SA, it will be submitted to the President of the Supervisory Board; in any other case a report is sent to the President of the Management Board of Sanok RC SA,

• the form guarantees the anonymity of the whistleblower; we do not collect any data identifying the whistleblower unless such data are included in the content of the report,

• if the whistleblower includes data allowing their identification in the report, we guarantee that they are protected in accordance with applicable laws and internal regulations concerning the protection of personal data,

• after the reported case has been explained, the data provided by the whistleblower allowing their identification will be removed in a durable way from any documentation stored in digital or physical form,

• in order for your report to be recognisable, try to describe the case in as much detail as possible.
